New Patient Forms
Upon entering our office Heather will greet you and welcome you as a member of our family. We will request you fill our patient forms located in the New Patient Center. The paperwork provides us with general information about yourself and your health issue or wellness goals.
Consultation - Day 1
Next you will have a consultation with Dr. Curtis to discuss your health issue and wellness objectives. Your first visit is designed for Dr. Curtis to learn more about you, your health issue and your expectations to determine whether you are a candidate for chiropractic care.
Physical Examination - Day 1
After your consultation, Dr. Curtis will perform a complete nerve system evaluation with our state-of-the art computer technology. This will be followed by a traditional chiropractic examination testing your ability to turn and bend as well as other standard neurological, orthopedic, postural evaluations.
Report of Findings - Day 2
Once all the information has been gathered and examinations have been analyzed, Dr. Curtis will give you a 'report of findings' to answer the three most common questions people have:
- Can Dr. Curtis help me?
- What will she recommend that I do to get better?
- How much is it going to cost?
All of our patients will have a written copy of their computer scans and examination findings to take home and review. Dr. Curtis uses this time to explain in detail the plan for the patient, and what the expectations are for the three phases of care.
Chiropractic Care
New Patients will generally be provided with their first Activator Methods Chirorpactic Technique adjustment at the conclusion of the Report of Findings appointment. The style of adjusting is extremely specific to help pinpoint the problem areas of the spine and nerve system. The adjustment is made with an instrument, so no manual forces are applied.

After Care Program
Prior to leaving, patients will be instructed on an after care program to be conducted at home. If you are in pain when you first come into our office this may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches. If you desire an overall wellness lifestyle, our wellness team will work with you to create good habits and routines that support your success. The foundation of our overall wellness lifestyle is maintaining healthy posture and good spinal alignment and function, eating healthy foods, taking vitamin/mineral supplements, keeping your weight under control and reducing stress .
