First Visit

Paperwork

Before entering the office, we have some paperwork for you to fill out. The paperwork provides us with general information about yourself and your health concerns. Please click here to download and print your paperwork. You can then fill it out before your appointment and bring it with you. New Patient Paperwork

Consultation

Once your paperwork has been reviewed, you will have a consultation with Dr. Curtis.
Your consultation is designed for the doctor to learn more about you, your health issue or wellness objectives and your expectations to determine whether you are a candidate for chiropractic care.

Examination (Computerized and Traditional)

After your consultation, Dr. Curtis will perform a complete nerve system evaluation with our state-of-the art computer technology. We use the Insight Millenium Subluxation Station. This will be followed by a traditional chiropractic examination testing your ability to turn and bend as well as other standard neurological, orthopedic, postural evaluations.

X-ray Studies

Your specific health concern may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective care plan for you. We refer out for these studies, and use the best tecnology available in digital x-rays. Dakota Radiology

Schedule Your Report of Findings (Visit 2)

You will be given a convenient appointment time for your Report of Findings visit. Generally speaking, patients are seen within 1-2 days.
Once all the information has been gathered and the examinations have been analyzed, Dr. Curtis will give you a 'Report of Findings' to answer the four most common questions people have:
  1. What is wrong with me?
  2. Can Dr. Curtis help me?
  3. How long is it going to take?
  4. How much is it going to cost?

All of our patients will have a written copy of their computer scans and examination findings to take home and review. Dr. Curtis uses this time to explain in detail the plan for the patient, and what the expectations are for the phases of care.
